1) The Internet (via the
college's website)
2) Paper Version (available in the Guidance Resource Center or
from the college itself)

(If you complete your application via the internet)
Turn the Following Items into the
Main Office (paper clipped together)
- Transcript Request Form
- with Application Checklist Completed
(available by clicking the link to the left or in the Guidance
Resource Center)
- Counselor/College Prep/Secondary
School Report
(when you apply online it should direct
you to print off this form, if the have one... not all schools use
this type of form)
- $2.00 for the Transcript
- Any Additional Materials That You
Would Like Sent To The College
- Activities & Achievements Resume
- Recommendations
- Personal Statement or Essay
Paper Version
(If you fill out the actual paper
application)
Turn the Following Items into the

- The Application Fee
(if there is one)
